This form is not for any of the following agencies: FBI, Honolulu Police Department, Active Duty military in Hawaiʻi, the Pentagon/ U.S. Department of Defense or any agency Headquartered on the mainland. Please send requests for the above agencies to their respective FOIA/UIPA websites.

This form is for FOIA/UIPA requests for the State of Hawaiʻi, Department of Defense and its divisions: Hawaiʻi Army National Guard, Hawaiʻi Air National Guard, Hawaiʻi Emergency Management Agency, Office of Veterans’ Services, and Youth Challenge Academy.
