Surety Bid Bond (Sample Form)

Form 1 – Certification of Bidder’s Participation in Approved Apprenticeship Program (Rev. 08/25/2010, pursuant to Act 17, State of Hawaii)

This certification form is required for bidders seeking preference under Act 17, which promotes the employment of apprentices in public works projects in Hawaii. It verifies a bidder’s active participation in one or more approved apprenticeship programs recognized by the State Department of Labor and Industrial Relations.

Corporate Resolution – Authorization of Officers

This Corporate Resolution form formally authorizes officers of a corporation to sign bids, proposals, contracts, and surety bonds on behalf of the company for work involving public agencies in the State of Hawai‘i or its subdivisions.

The company officer must sign the form and have it notarized to show compliance with state hiring requirements.
